Membership application forms

When the employer first enrols in CONNECT, a signed membership application form for each new member (employee) should be attached to the Employer Application Form and returned to AAS.

Please check the application forms to ensure they are filled in correctly before sending them to the AAS. Note that a new membership application cannot be accepted unless it is signed.

Subsequent new application forms should be attached to the monthly contribution form when these are returned to AAS.
